Post-Operation Notes

Procedures
- Patient given 2 x 250mg Ponstan preoperatively.
- Left inferior dental nerve block by administration of 2% Xylocaine 1:80000 Adr one cartridge. Half a cartridge for long buccal nerve and half a cartridge for lingual nerve block.
- Modified Ward’s Incision made from the mid of 37, carried posteriorly in the crevicular sulcus until reaches 38 and incision continues distally towards external oblique ridge for 1 cm. A relieving incision is made at the bucacal side of 37 until about 5 mm beneath the attached gingiva.
- Mucoperiosteal flap is raised using Howarth’s periosteal elevator.
- Gutter at the depth of the bur is made over the buccal and distobuccal aspect of 38
- Tooth sectioning is done, from the mesial cemento-enamel junction of 38 upwards to the White line. Weaker coronal part is fractured off electively wit elevator and removed.
- A forcep is then positioned over remaining tooth structure and delivery of tooth is done. Toilet of wound from remaining dental follicle, bony spicules, etc. A bone file was used to smoothened sharp bony edges.
- Haemostasis achieved.
- Flap repositioned with 3’0 silk (3 stitches)
- Post- operative instructions given. Patient asked to return for review and removal of sutures on 2 October 2006.
